Hebrew Rehabilitation Center in Dedham, MA is hiring for per diem, part time, and full time LPNs for our Long Term Care and Rehab units!

Hiring for day, evening, and night shift - shift differentials included.

Job Responsibilities

1. Ensures all care delivered is within the scope and established standards of safe professional nursing practice and HSL core competencies

2. Collaborates with other members of the clinical support team (MD, NP, nurses, social worker, therapists, dietitian) to meet the clinical needs of hte patient and monitor outcomes of care

3. Practices in a primary nursing model and maintains accountability for a set of assigned patients

4. Collaborates and participates in care planning activities and supports autonomy and individual choice wherever possible

5. Documents appropriately in the electronic medical record

6. Delegates and supervises clinical care delivered by the nursing assistant

7. Maintains compliance with policies, proceduresm, practice and regulatory matters

8. Parcipate and assists with activities (i.e. assist with serving meals)

9. Participates in the collection and monitoring of quality data to ensure high standards of care

Required Qualifications

1. Graduate of approved school of nursing required

2. Current Massachusetts license as an RN or LPN in good standing

3. Understand the philosophy and support the model of care at HRC required

4. Computer literacy required.

5. Excellent English language skills, written and verbal.

Job Types: Full-time, Part-time

Pay: $30.00 - $45.00 per hour
