Constellation Health is looking for compassionate and empathetic Social Workers to work with our Hospice patients. This role will involve providing care in the patients' homes, assisted living or skilled nursing facilities. Ideal professional we be a licensed clinical social worker or a licensed master's clinical social worker.

Schedule: PRN, Saturday Optional

Territory: Philadelphia lower Bucks County ( North East Philadelphia, Somerton, Bensalem and Levittown)

This Is You:

• Bachelor Degree in Social Work a must! LSW or LMSW in the state of Pennsylvania with at least 1 year of clinical experience, hospice experience desired
• Have reliable transportation and willing to commute within a designated territory to provide the best in home health service.

Job Responsibilities:

• Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, neglect or abuse and plan intervention based on evaluation findings
• Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan
• Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record keeping system. Submits all clinical documentation, time records and other information in the required time frame
• Occasional weekend coverage & holidays (as needed)

Together, we:

• Provide Clinical Social Work to Hospice population. Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, neglect or abuse and plan intervention based on evaluation findings
• Educate patients and caregivers on 28 medications, plan of care and individualized treatment plan
• Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record keeping system. Submits all clinical documentation, time records and other information in the required time frame
