SUBMISSION REQUIREMENTS
- TN LPN license - REQUIRED
- Graduate of a state approved vocational education program - REQUIRED
- 1 year experience - REQUIRED
- COVID vaccination required **Please add vaccination status in the submission notes**
- Only 1 reference needed at time of submittal
