JOB SUMMARY:
The MAT Program Coordinator is a member of the integrated behavioral health team. Position is responsible for engaging and supporting patients receiving Medication Assisted Treatment (MAT). The Program Coordinator works with the patient, family, provider, and health care team to coordinate care resources and services to improve the quality and effectiveness and decrease the cost of patient care. The Program Coordinator also works closely with local resources such as the local emergency departments to coordinate care. This position is responsible for directing and supervising the work of the Peer Recovery Coaches. This is a full-time position requiring 40 hours a week.
CHARACTERISTIC, DUTIES AND RESPONSIBILITIES:

- Performs key functions related to the success of the MAT program within River Valley including patient outreach via telephone, community outreach and office encounters, coordination of patient activities, and the appropriate utilization of services.
- Maintains regular communication with patients in assigned caseload via assessed model of care. Includes the patient’s family, caregivers, and other support in care coordination activities as appropriate.
- Ensures effective collaboration and communication between the patient and members of the healthcare team to support care coordination activities and goal attainment, including but not limited to local emergency rooms and hospitals.
- Ensures patient, family and caregiver understanding of condition management, medications, tests, referrals, and harm reduction model services.
- Coordinates care across the continuum by working to close care gaps.
- Provides post-discharge follow up calls to hospitalized patients, acts as a liaison with specialty clinics, addresses and works to resolve patient concerns or barriers to treatment plan adherence, and provides a link to appropriate community resources.
- Facilitates patient access to resources including financial assistance.
- Identifies and implements strategies such as motivational interviewing to promote patient engagement, self-care and optimal levels of health and well-being.
- Facilitates inclusion of the patient in ongoing decisions regarding the plan of care.
- Provides or facilitates education necessary to support timely and informed decisions and adherence to the plan of care.
- Assesses and identifies strategies to improve the health literacy of the patient and, as appropriate, family, and other caregivers.
- Monitors effectiveness of the plan of care and collaborates with the patient, family, and caregivers to revise as indicated.
- Serves as a patient advocate. Consults with providers and other team members as appropriate to resolve care issues.
- Documents all patient care activity; enters information into the appropriate system and maintains the patient registry.
- Maintains patient registry and regular data monitoring including data entry of patient data into outside platforms.
- Aware of and responsive to cultural and demographic diversity of the population.
- Acknowledges patient’s rights and maintains patient confidentiality at all times and maintains documentation in compliance with agency, HIPAA, 42CFR, and funder standards
Additional duties and responsibilities:
- Collaborates and communicates with stakeholders regarding improvements, data entry, treatments assessments and the success of the MAT Care Coordination Program.
- Participates in grant funded activities specifically around MAT, including data entry (also in outside data platforms), data monitoring in coordination with data team
- Schedule follow-up appointments after induction into MAT program
- Attend community care plan meetings with Montrose Memorial Hospital surrounding frequent utilizers of ED
- Assists MAT patients with scheduling issues, no-shows, reschedule requests, same-day status
- Facilitates obtaining medication for in office inductions
- Monitors patient progress, attendance, medication requests/refills
- Establish working relationships with community partners such as Hilltop, Montrose Memorial Hospital, Delta Memorial Hospital, Center for Mental Health, Advocates for Recovery Colorado, Oxford House, 7th Judicial District
- Identify RVFHC patients who may benefit from harm reduction methods
- Provide harm reduction education and supplies to patient and clinic staff

- Perform such other duties or projects as determined by this position’s supervisor

JOB QUALIFICATIONS:
Education or Formal Training & Experience:
- CAS Certification required
- BLS/CPR certification required
- Drug/alcohol certification preferred
- At least one year of experience in an ambulatory health care sitting desired.
- At least two years working with the SUD population in a clinical setting, outpatient setting, or care coordination experience is preferred.
Knowledge, Skill & Ability:
- Ability to establish and maintain effective, courteous working relationships with patients and staff team members.
- Ability to work under pressure in a fast-paced environment.
- Ability to travel to meetings outside of the service area.
- Ability to work flexible hours to meet job requirements
- Comprehensive knowledge of community resources and functions as a liaison to facilitate patient access.
- Bilingual in Spanish/English preferred.
Physical Requirement and Workplace Environment:
- Requires periods of standing and walking
- Requires long periods of time sitting while on the telephone and/or doing computer work.
- Requires sufficient near vision to be able to read documents and computer screen.
- Essential to have ability to lift, carry, push, and pull up to 35 pounds.
- Essential to have ability to use routine office equipment such as computer and telephone.
- Essential to have ability to stoop, kneel, bend, crouch, twist, and reach.
- Essential to have ability to hear routine conversations.
- Essential to have ability to comprehend both oral and written communications.
- HIPAA Classification: Unrestricted Access: A workforce member with unrestricted access will have full access to patient’s protected health information, including the patient’s entire medical record, for patient care purposes.

The above health, dental, and vision benefits are available by employee choice to all full-time employees after 60 days of employment effective the following first of the month. The option for the 401(k) is available after 3 months of employment. All other benefits listed are active as of the date of hire.
