Maternity Case Manager

Do you have a heart for moms and babies? Good Shepherd Children & Family Services Maternity program is looking for a caring, dedicated professional to join our Maternity Shelter & Transitional Living Program, which provides intensive, 24-hour care for teen and young adult moms and their babies facing homelessness. Good Shepherd offers a generous benefit package and competitive compensation, with case manager wage ranging from $40,000 to $45,000 per year depending upon experience.
Primary duties:
This is full time position. The Maternity program case manager is responsible for linking residents to a variety of services and resources to meet their needs across all life domains and for coordinating the efforts of all external providers involved in residents’ care so that the treatment plans are supported. This position involves assessing and addressing the individual and family needs of our pregnant/parenting residents, as well as providing structured programming to enhance their life skill mastery. It includes collaborating with other programs and organizations while maintaining a client-centered approach. This program is a 365/24/7 endeavor and this position requires flexible work hours that may include shared after-hours program support.
Qualifications for position:
Required Bachelor’s Degree in social work, psychology, or counseling, and at least two years of successful, paid experience in a similar setting in a capacity which allowed for relationship-building and/or case coordination. Preferred Master’s Degree in social work, psychology, or counseling. Familiarity with the impact that teen parenting, trauma, abuse, chronic homelessness and family discord can have on youth, and ability to recognize and encourage resiliency.
