MDS Manager
John Knox Village is seeking a experienced RN to be our MDS Manager to ensure appropriate reimbursement for Short Term Rehabilitation Guests. This is a full time position Monday- Friday. As the MDS Manager, you manage a team of RNs and will treat residents and their families with respect and dignity, ensuring resident confidentiality and privacy.
You will be responsible for the following:
- Minimum Data Set (MDS) assessments are completed accurately and within the required time period.
- Weekly, collaborate with Rehab Manager, Admissions, and Medicare Biller to discuss all residents currently on Medicare, potential residents, and pending discharges. Discuss potential changes in RUGs category, and change of conditions and residents progress toward goals.
- Management of MDS and PPS scheduling and planning.
- Review of MDS assessments for accuracy and correct coding, modification if necessary, and locked timely.
- Ability to perform triple check review to ensure all significant MDS RUG levels were included and with appropriate ICD-10 Diagnoses Codes for billing. Responsible for notifying the billing office of all changes in the RUG levels.
- Develop and revise plan of care as indicated by the resident's response to treatment and the evaluation of the effectiveness of the overall plan of care.
- Facilitate care plan meetings according to standard of practice.
- UB Review. Review all billings. Maintain Triple Check to assure for accuracy.
- Perform a nursing physical and psychosocial MDS assessment on as needed residents and reassessments as per the MDS schedule requirements.
- Oversee the utilization of treatment, therapy and nursing interventions, and use of supplies and equipment.
- Oversee the assignment of resident to Resource Utilization Groups (RUGs) and the payment periods associated with the MDS schedules.
- Oversee the resident assessment dates, plan of care, and assignment of RUG classifications by the MDS Team.
- Demonstrate knowledge of the Minimum Data Set 2.0, Resource Utilization Groups (RUGs), Triggers, Resident Assessment Protocol Summary (RAPs), Plan of Care, Discharge Plan, and documentation requirements per regulations and legislation.
- Manage and collaborate with the MDS Team on look-back periods and informs the interdisciplinary team regarding changes in the look-back periods for the MDS completion including the "significant change" MDS.
- Oversees the Management the MDS tracking log.
- Random review of MDS's completed by RN and review/sign MDS's completed by LPN to ensure correct coding, edited if necessary, and locked by required time frames.
- Attend the care plan meeting and assist with resident and family concerns.
- Perform 10 full chart audits a month. 5 Polaris audits will be reviewed and a plan of action will be implemented.
- Provide staff teaching as required. Ensure education of Point of Care. Monitor for improvement and maintains ongoing audits of Point of Care.
- Ensure employee licenses, certifications and PPD documentation are current and compliant with AHCA, and copies are maintained in the employees' HR files.
- Separation Forms are received in the Human Resources Department within 24 hours of employee's separation of employment date.
- Employees receive their 90 day and annual performance evaluations by their due date.
- Utilize company technological resources (project management programs, smartsheets).
- Perform other duties as deemed necessary and appropriate, or as may be directed.
We want to speak with individuals who has the following:
- 4+ years geriatric nursing management or case management experience.
- RN Licensed in the State of Florida.
- Knowledge of federal, state and local government regulations and their impact on health care services.
- Knowledge of the MDS 3.0, RAI rules and regulations and financial implication of PPS.
- Intermediate skills with Microsoft programs, project management tools, Point Click Care.
- Strong organizational skills.
