Position Overview

The Medical Case Manager is part of the Vulnerable Populations Support Team (VPST) and is responsible for assisting patients in gaining access to needed medical, social, educational, and other support services. This position will work on and off site with extremely vulnerable patients to coordinate services both within 4th Street Clinic and among other community providers. Case management services will include referral and follow-up services for medical and behavioral healthcare, insurance eligibility, transportation, care gap analysis, and improving patients’ social determinants of health.

Core Responsibilities

  • Collaborate with members of the VPST and primary care team to identify patients that need case management.
  • Conduct a needs assessment for patients identified as medically vulnerable.
  • On a regular basis, develop and update a patient centered care plan that builds upon areas of need identified by the case management needs assessment.
  • Coordinate the delivery of medical, behavioral health, and supportive services for patients within the clinic and with community partners.
  • Provide patient transportation in agency vehicles when needed.
  • Collaborate with other service providers to ensure continuity of care for patients.
  • Assist patients with linkages to other community organizations and service providers who can help meet patient needs.
  • Serve as the primary point of contact for patients transitioning to and from institutional settings such as jail or hospitals to ensure continuity of care and coordinated reentry.
  • Maintain knowledge of clinic policies, procedures, systems, and electronic health record.
  • Maintain case management data to facilitate reporting and the monitoring of quality.
  • Maintain accurate, up to date records in the clinic's electronic medical record and ensure accuracy for billing.
  • Other duties as assigned.

Qualifications

  • High school diploma or equivalent
  • Three years of Social Service or Community work experience
  • Experience working with homeless or vulnerable populations.
  • Complete and maintain a Utah State Certification for Case Management.

Preferred

  • No degree but has five years of experience in Medical Case Management, Intensive Case Management, or Care Coordination experience
  • Associate’s degree in Social Work, Social Services and similar degree with 3 years of experience in field
  • Bachelor's degree in Social Work, Social Services, or similar degree with 1 year of experience in field
  • Social Service Worker (SSW) Utah license

Job Type: Full-time

Pay: $20.00 - $22.00 per hour
