Overview:
Responsible for assessing prospective enrollees and active health plan members to determine potential care needs and plan eligibility.
This position is on a part-time, as needed basis. Assessments will be conducted in the member's home, according to their schedule.
Responsibilities and Standards:
- Makes virtual or in-person visits to potential enrollees and active members of health plan as assigned.
- Provides an overview of program benefits to an applicant, family member(s), and caregiver(s).
- Informs consumers and members of their rights, responsibilities, and services.
- Interviews members, family members, and/or significant others identified through case finding to assess needs for services.
- Reviews the member handbook, provider directory, and services covered during the visit.
- Reviews and evaluates previous assessment information.
- Performs all assessment activities in compliance with all regulatory agency requirements; completes required documentation and develops the initial or updated plan of care.
- Assists in determining unmet needs to be met by creating a person-centered service plan. Design a person center service plan driven by assessment determining appropriate and necessary services to meet unmet needs.
- Communicate effectively with potential members, their families, care management staff and providers.
- Collaborate with the health care team regarding potential enrollee’s needs.
- Collect information regarding eligibility, benefit coverage, and coordination of benefits.
- Scan in all pertinent documentation to the EHR system
- Input visit and assessment data into required EHR systems
- Participate in departmental meetings and attend all scheduled staff meetings and training as applicable.
- Adhere to all state and federal privacy regulations, including HIPAA and Crane Home Care policies and agreements regarding confidentiality, privacy, and security.
- Maintains accurate scheduling information
Requirements:
- Nursing: License and current registration as a Registered Professional Nurse in New York State.
- Education: BSN preferred; Associate degree required.
- Long-term care and/or managed care experience required.
- Knowledge of homecare, Medicare, and Medicaid preferred.
- Ability to work independently and prioritize workload.
- Computer literate and working knowledge of basic computer skills and Microsoft Office programs.
- Strong communication skills person to person and by phone.
- Valid NYS driver’s license and ability to drive in all types of weather.
- Ability to learn the organization’s software system to perform job duties.
Physical Requirements:
- Standing, sitting, bending, walking, and driving (frequently entering and exiting vehicle throughout the day) are standard postures.
- Frequently uses a laptop computer to enter orders and obtain product information.
- Sensory requirements include speech, vision, smell, and hearing.
- Lifting requirements of up to 20 pounds frequently and up to 50 pounds occasionally
- Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.
