RN - Nurse Navigator - Comprehensive Stroke Center

Job Description:
Shift: Full - Time, (Monday - Friday, 8am - 4pm)
General Description:
Will be responsible for communicating and coordinating programmatic workflows and opportunities with system, hospital, partner sites, and community stakeholders to provide care for the stroke patient population
Essential Responsibilities:

  • Coordinates patient care throughout the continuum of care in collaboration with the multidisciplinary team.
  • Serves as a liaison with hospitals that are part of the OU Health tele-stroke network throughout the state of Oklahoma and the region.
  • Travels as needed to various sites to assist tele-stroke partners within the tele-stroke network
  • Ensures that OU Health and partner hospitals are satisfied with all components of the program.
  • Responsible for stroke education outreach with internal and external partners as well as community stroke education and awareness activities.
  • Supports leadership and providers in committee meetings, process improvement projects and gathering of relevant data for analysis and report out.
  • Uses clinical expertise and management skills to direct education and services to ensure the delivery of high-quality care in the most family-centered, efficient and economical manner.
  • Works in collaboration with internal and external interdisciplinary team members to achieve excellence patient service throughout the plan of care.
  • In collaboration with the management team, is responsible for planning, implementing, evaluating and development of education related to tele stroke.
  • Utilizes advanced practice expertise to propose, develop and implement processes with the goal of improving the clinical experience for the patient.

Minimum Qualifications:
  • Education: Graduate from an accredited school of nursing required. Bachelor of Science (or higher) in Nursing Board Approved Program preferred.
  • Experience: Two (2) years of experience as an RN in an acute care setting or One (1) year of acute emergency services
  • License(s)/Certification(s)/Registration(s) Required: Current RN License issued by the Oklahoma State Board of Nursing, or a current multistate compact RN License (eNLC). Current Basic Life Support and current Advanced Cardiac Life Support issued by the American Heart Association required upon hire.
  • Knowledge, Skills and Abilities:
  • Knowledge of the principles of supervising staff.
  • Skill in planning and organizing nursing care, applying advanced nursing clinical skills, maintaining records and charts, and preparing written reports.
  • Skilled in maintaining effective working relationships with others.
  • Excellent verbal and written communication skills.
  • Ability to work effectively as part of a team.
  • Knowledge of nursing theory, practices, techniques, procedures and standards.
