At AVP CCPHP, we believe that strong doctor-patient relationships are essential to helping you achieve optimal wellness. The concierge (membership-based) model at AVP CCPHP allows patients to build a strong rapport with Dr. Vishal Patel and collaboratively develop a personalized approach to their health. Members are guaranteed a more convenient, connected, and collaborative relationship with Dr. Patel.

In addition to Dr. Patel’s concierge program, he continues to oversee the care of numerous non-Member patients via his practice, First Med Health. We are currently looking to fill a full-time Nurse Practitioner to deliver high-quality primary care to our patients who are not part of Dr. Patel’s concierge program. As an NP you will be responsible for the delivery of primary care for patients under the mission and vision of Dr. Patel and First Med Health. This position is very autonomous and independent; you must have advanced Primary Health Care knowledge, be confident in your decision-making, be exceptional in your client-service approach, and be experienced in caring for complex conditions.

Responsibilities:

• Oversee the administration of day-to-day primary care for patients
• Provide high-quality customer service
• Manage and treat complex chronic comorbidities
• Collaboration with an interdisciplinary team in the course of care delivery
• Collaborate with Dr. Patel as the overseeing physician and on referring patients to the possibility of the concierge membership program

Job Requirements:

• Completion of a recognized Nurse Practitioner program - Primary Health Care required
• Minimum 3-5 years of relevant clinical experience (geriatrics preferred)
• Ability to deal with crisis situation promptly and effectively
• Exceptional communication and customer service skills
• Strong interpersonal, leadership, and problem-solving skills
• COVID Vaccine practitioner: 1 year (required)
• Valid CPR and First Aid certification
• Current vulnerable sector police check
• You must have reliable means of transportation
• Current extended Nursing class certification preferred
• Experience with chronic disease management and geriatric care
