Job Description:

What we’re looking for in you

Required:

• Graduate of an accredited Masters of Nurse Practitioner program.
• Ability to meet the requirements to prescribe medications according to the Minnesota Nursing Act
• Current MN or WI Certified Nurse Practitioner (CNP) license by the Minnesota Board of Nursing (MN) or the Wisconsin Department of Safety and Professional Services (WI) upon start date.
• Must provide proof of MN and WI CNP license within 4 months of start, if both MN and WI CNP licenses are not present on start date.
• Current Adult, Adult-Gerontology Primary Care or Family Nurse Practitioner as recognized by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP) active on the Minnesota Board of Nursing.
• Current BLS certification through American Heart or Red Cross required for Hospice Nurse Practitioners working palliative care (Hospice NP’s working in hospice care and not palliative care are not required to have BLS)
• Must have a valid driver’s license and meet all criteria required under Lakeview’s Automobile Liability Guidelines.

Preferred:

• Proof of application for alternate state within 4 weeks of start, if both MN and WI CNP licenses are not present on start date.

The skills/experience you bring as a Palliative Care Nurse Practitioner

Required:

• Requires 5-8 years of experience as a Registered Nurse
• Demonstrated commitment to health promotion, patient education, and a collaborative approach to health care services
• Demonstrated ability to work with a multi-disciplinary team
• Excellent verbal and written communication skills, effective time management and problem-solving skills
• Knowledge of health care management in the Homecare/Hospice/Palliative Care environment
• Accept responsibility for maintaining current knowledge level of nursing trends in the Hospice field
• Ability to read, write and speak the English language

Preferred:

• 2-3 years of experience as a Nurse Practitioner in a Homecare/Hospice/Palliative and/or geriatric setting

The purpose of the Palliative Care Nurse Practitioner role

As a Palliative Care Nurse Practitioner you have a delegated responsibility for the medical component of patient care for Lakeview’s Community-Based Palliative Care Program in collaboration with the medical director and clinical manager for Community-Based Palliative Care.
