Job Description:
SoCal Premium Hospice Care is currently seeking a full-time Nurse Practitioner to join our team in the Palmdale area. The Nurse Practitioner will provide high-quality hospice and palliative care services to patients in their homes or in long-term care facilities, in collaboration with a team of interdisciplinary healthcare professionals.
Responsibilities:
Conduct comprehensive assessments of patients and develop individualized care plans in collaboration with the interdisciplinary team
Manage and coordinate care for patients with complex medical needs and symptoms, including pain management and symptom control
Monitor and evaluate the effectiveness of the care plan and make adjustments as needed
Provide education and support to patients and families on hospice and palliative care, disease process, and end-of-life care options
Communicate and collaborate effectively with other healthcare professionals to ensure continuity and quality of care
Maintain accurate and timely documentation of patient assessments, care plans, and interventions
Participate in quality improvement initiatives and ongoing education and training
Requirements:
Current California license as a Nurse Practitioner
Minimum of 2 years of experience in hospice or palliative care
Knowledge of hospice philosophy, principles, and practices
Strong clinical and assessment skills, with the ability to manage complex medical needs and symptoms
Excellent communication and interpersonal skills, with the ability to work effectively as part of a team
Proficient in using electronic medical records and other relevant software applications
Must have a valid driver's license and reliable transportation
