Nurse Practitioner (Direct Patient Care / Home Health)

Full-Time / Up to $125k/year

Connected Health Care, a leading healthcare recruitment company, is seeking a Nurse Practitioner to join a hospital in Colorado Springs. In this role, you will provide direct patient care as well as home health/hospice services. This is a full-time position with competitive compensation and benefits.

Responsibilities:
Provide direct patient care in the hospital setting, including physical exams, diagnostic tests, and medical treatment plans.

Collaborate with physicians, nurses, and other healthcare professionals to provide comprehensive care to patients.

Provide home health and hospice services to patients, including assessments, evaluations, and care planning.

Educate patients and their families on healthcare issues, treatments, and self-care techniques.

Maintain accurate and timely medical records and document patient care.

Requirements:
Master's degree in Nursing and certification as a Nurse Practitioner

Current Colorado state license to practice as a Nurse Practitioner

Minimum of 2 years of experience in direct patient care

Experience in home health/hospice preferred

Strong clinical skills and ability to work independently and as part of a team

Excellent communication and interpersonal skills

Ability to work in a fast-paced, dynamic environment
