Integritus Healthcare's (formally Berkshire Healthcare Systems) newest affiliate, East Longmeadow Memory Care Assisted Living Center opened in August 2022. This 20 bed residence is just steps away from the state-of- the-art Skilled Nursing Center which was built in 2020 providing a supportive Campus of Care for the local community. Our memory care assisted living community provides a unique environment where residents receive personal assistance with activities of daily living in a supportive environment for one price that does not escalate with higher care needs.

At East Longmeadow Memory Care Assisted Living, our Life Enrichment Memory Care Program (LEP) is designed for those who need support with their daily activities such as bathing or dressing. When care needs progress, residents may transition to our certified memory care neighborhood at the Skilled Nursing Center next door. Our secure unit is designed to create a familiar transitional environment from the assisted living to long term care in order to minimize confusion and provide comfort.

East Longmeadow Memory Care Assisted Living and Skilled Nursing Center are conveniently located on the border of Longmeadow and East Longmeadow at 305A & 305C Maple Street, East Longmeadow.

Job Description

Case management of health-related assistance-in-living services. Coordination and supervision of all health-related services 24 hours per day 7 days a week. Has knowledge of resident Bill of Rights and assisted living ethics. All responsibilities will be conducted in a manner that exhibits the BHCS mission, vision and core values.

Experience: 2 year working in a community-based setting preferred.

Education and Training: Graduate from an accredited school of nursing.

License, Certification and Registration: Licensed Practical Nurse (LPN) with the Massachusetts Nursing Board.

Job Type: Part-time

Pay: From $30.00 per hour
