Home Health Occupational Therapist in Queens/Nassau, NY

Responsibilities:

• Initial evaluation of patient’s functional status in a home setting. Develop and implement appropriate care plans for patients. Monitor patient progress and document changes in condition. Provide patient and family education.
• Development and implementation of a care plan Coordinating with other health care providers. Assessing the need for additional services or modifications to the plan of care.
• Ongoing one-to-one treatment in the home environment. Regular monitoring of progress and adjustment of the care plan accordingly. Regular meetings with the patient and other involved parties.
• Educate patients and caregivers on the care plan and individualized treatment plan. Ensure compliance with the prescribed treatment plan. Provide emotional and psychological support to the patient and their family. Regularly check up on the patient's progress and adjust the care plan accordingly.
• Maintains and submits all clinical documentation in compliance with Agency standards. Monitor the patient's response to treatments and medications. Report any changes in the patient's condition to the physician. Provide follow-up counselling and support when necessary.

Requirements:

• Occupational Therapy License in New York State
• Minimum one year of clinical practice. Home Care Preferred
• Valid NYS Drivers License and reliable transportation
• Travel throughout the Borough of Queens

Job Type: Full-time

Salary: $89,322.09 - $91,248.66 per year
