Overview: SYNERGY HomeCare services people of all ages, providing only the best companionship, homemaking, and personal care services in the communities of Woodbury, MN and River Falls, WI. Our caregivers are responsible for companionship (conversation, planning activities, medication reminders, maintaining a safe environment, hospital sitting, etc.), homemaker activities (light housekeeping, grocery shopping, running errands, meal preparation, etc.), and personal care (bathing, grooming, lifting, etc.).

Job Description: The On-Call Caregiver is expected to perform a variety of duties and be able to work with all clients, caregivers, and office staff. This person will report to the Owner and is expected to help provide the highest quality of care for our clients and be a brand ambassador. This person needs to have flexibility in their calendar and the confidence to jump into any caregiving or office task. With reviews happening at 30-, 90- and 1-year review upon hiring.

Guarantee of hours: This means you will work when shifts are available between the hours of 6a-6p Monday through Friday. You are also required to be available to work 2 weekends a month. The goal of this role is to ensure coverage for all clients when staff is unable to make normally scheduled shifts. You have one hour to report to a shift from being notified by the office. This position reports to Alicia Harmon.

Responsibilities:

• Help clients with daily activities, such as bathing and bathroom functions, feeding, grooming, taking medication, and some housework.

• Help clients make and keep appointments with doctors, provide or arrange transportation and serve as a companion for their clients.

• Communicate and interact in a professional, respectful, and hospitable manner.

• Assist with daily personal and medical care routines according to individual care plans.

• Provide physical fitness, brain fitness, and social/spiritual enrichment activities.

• Observe residents and report to RN with any changes in physical, mental, and emotional condition.

• Record proper documentation per established procedures.

• Ensure proper cleaning and sanitation of equipment and living areas.

• Promote teamwork, laughter, and happiness every day.

• Establish and maintain healthy boundaries.

• Be comfortable with a varied daily shift and traveling up to 25 miles.

• Cover caregiving shifts due to call-offs or last-minute needs.

• Communicate with Office Team, Caregivers and Clients as schedules change.

• Office phone back-up as needed.

• Introduce new clients and caregivers in the field.

• Field networking (post 30 days)

Required:

• Compliant with all training requirements for the state of MN

• Compliant with Policy and Procedure manual per MN licensing requirements and SYNERGY HomeCare specifics.

• Valid driver’s license, car insurance and a reliable car

• TB screening
