Job description
Serves as a single point of contact for total joint replacement patients and their caregivers to provide resources and assistance with accessing clinical and supportive care services. Promotes optimum clinical outcomes and insures appropriate utilization of covered services by making it easy for the member to receive the services required to treat the members condition(s) and comply with the physicians care plan. Transitions between care settings (e.g. inpatient, outpatient and other settings), cross market coordination, chronic disease management complex case management and promotion of preventive services. Practices professional nursing by the performance of independent nursing functions in accordance w/accepted practice standards, including the initiation and performance of nursing care through health promotion, supportive or restorative care, disease prevention and nursing treatment of human disease, ailment, pain, injury,deformity, and physical or mental condition using specialized knowledge.

Essential Responsibilities:
  • Provides assessment and management of critical transitions from hospital to home and LTC/SNF to home using evidence based guidelines that ensure a safe transition for the member.

Basic Qualifications:

Experience
  • Minimum three (3) years of experience in care coordination and disease management for medically complex patients, experience in utilization review, case management, and discharge planning required.


Education
  • Associates degree or diploma in Nursing.


License, Certification, Registration
  • This job requires credentials from multiple states. Credentials from the primary work state are required before hire. Additional Credentials from the secondary work state(s) are required post hire.
  • Registered Nurse License (Maryland) within 6 months of hire AND Registered Nurse License (Virginia) within 6 months of hire


OR

  • Compact License: Registered Nurse within 6 months of hire
  • Registered Nurse License (District of Columbia) within 6 months of hire


Additional Requirements:
  • Local travel required to network and KP facilities to communicate, develop relationships of trust and obtain critical patient/member information to inform the development of a care plan for the member.
  • Working knowledge of case management which includes but is not limited to competencies in the following: application of motivational interviewing skills, development and documentation of an integrated multi-disciplinary care plan, collaborating and negotiating with team members and family members, accessing internal and external resources.


Preferred Qualifications:
  • Orthopedic experience preferred.
  • Bachelors degree or Advanced degree in Nursing.
  • Familiarity with Interquel and/or Milliman criteria preferred.
