Position Summary:
Under the direct supervision of the Housing Partnerships Manager, the Housing Service Coordinator is responsible for identifying and enrolling potential PACE participants residing in housing sites. The PACE Housing Service Coordinator provides oversight and coordinates housing site activities and services. The Housing Service Coordinator serves as a liaison between the Interdisciplinary Team, (IDT), administration, property management and other service providers for the housing site.

Essential Functions of the Job:
  • Responsible for identifying and ensuring enrollment of potential PACE participants in housing sites and maintains accurate records of referrals.
  • Develops and maintains a professional relationship with Property Management and the facilities personnel.
  • Coordinates with the PACE Inter Disciplinary Team (IDT) as needed.
  • Assists in the coordination of PACE services when needed. Services include but not limited to: Care planning, preparation for interdisciplinary conference, daily meetings, family conferences, transportation, appointments, medication delivery and assessments.
  • In collaboration with property management, ensure that all services and activities on-site comply with state regulations and guidelines.
  • Maintain appropriate level of communication and contact with PACE participants that are residents of the housing site.
  • Serves as a liaison between disciplines, administration, PACE participants and other direct staff serving the housing site.
  • Create a PACE calendar and distribute to all potential and existing PACE participants at housing site.
  • Coordinate with SD PACE marketing department to promote PACE services at housing sites.
  • When appropriate, coordinates with external partners to provide key services or activities to housing residents.
  • Is knowledgeable of community resources and social services that could potentially assist the residents of the housing site.
  • Conducts residential activities in the community space as needed and maintains accurate records of resident participation in an effort to elicit new enrollments into PACE.
  • Manages and works with a budget and regularly communicates with Housing Partnership Manager of any extenuating need.
  • Documents all encounters with participant in TruChart and adds appropriate reviewer when applicable.
  • Maintain a safe, secure, and legal work environment; develop personal growth opportunities.
Additional Duties and Responsibilities:
  • Provide oversight and coordinates housing site activities and services
  • Performs other duties as assigned.
  • Serve as a representative of PACE in the community and maintains a high level of professionalism throughout.
  • Ability to work independently, display leadership skills and takes initiative on site.
Job Requirements: Education Required:
  • High School Diploma
Education Preferred:
  • Bachelor’s degree in health related or social service field
Certifications/Licenses Required:
  • CA valid driver’s license and reliable transportation
  • Proof of current vehicle insurance (if applicable)
Experience Required:
  • Minimum of 1-2 years in a healthcare, housing or social service setting.
Technical Knowledge and Skills Required to Perform the Job:
  • Must possess the skills necessary to provide care for geriatric patients, must thrive in team environment, must possess good organizational and supervisory skills, ability to effectively handle difficult and unusual interpersonal situations. Meet a standardized set of competencies for the specific position description established by SD PACE and approved by CMS.
Working Conditions and Physical Requirements:
  • Physical clearance for reaching, bending, stooping, crouching, kneeling and grasping; have full range of motion; ability to lift 50 lbs; need to lift and transfer participants; T.B. clearance.
