Full Job Description:
General Summary of Duties:
The Patient Access Specialist is a front-line customer service role. Responsible for coordinating the delivery of quality care and services and is an essential part of the primary care team. The primary focus of this role is assisting patients with community resources, medical records, obtaining a primary care provider, and general information. Must have a self-starting personality, enjoy patient care, and desire to enhance patient relations while maintaining efficiency.

Primary responsibilities:
Works closely with NBMC primary care, internal medicine, and specialty providers along with community resources to assist in the coordination of care for patients of all ages with complex and chronic health care needs. Provide support, education and information to patients and providers. Schedule new patient appointments upon new provider acceptance. Triaging patient concerns/problems. Set up new patient accounts, verify insurance and demographics. Responsible for overflow check in process including maintaining a cash drawer and balancing funds at the end of each shift.
As the Patient Access Specialist, you will be Assisting the feedback coordinator with patient and provider feedback. Patient Access Specialist is responsible for receiving complaints and corresponding with patients and providers to resolve conflict. Documenting and filing grievances/complaints. Responsible for follow-up via phone or mail. Communication with providers, medical staff, administration, and insurance companies when necessary to resolve complaints.

Essential responsibilities:
  • Facilitate primary care provider assignment.
  • Assist patients with records release, Release of information forms, and intake forms.
  • Participation in quality improvement activities.
  • Maintaining spreadsheets for quality improvement data and tracking
  • Facilitate Advanced Directives and POLST paperwork.
  • Assist patients with MyChart support.
  • Other duties and tasks as assigned.
