Job description
The Patient Care Coordinator coordinates the care of patients receiving total joint replacement and hip fracture repair surgeries. In addition, provides follow-up and care across the continuum from pre-surgery to post operatively. The Patient Care Coordinator collaborates with members of the health care team to enhance patient care. This position also serves as the primary point of contact for these patients and their family members.
The degree of supervision required for the performance of any delegated activity by the patient care coordinator when implementing care is determined by variables which include, but are not limited to:
Ability to communicate effectively with physicians and staff both verbally and in writing.
Ability to communicate effectively with patients and family members.
Adherence to all HIPAA and patient rights policies.
Knowledge of orthopedic procedures and policies.
Strong organizational skills.
Ability to handle multiple tasks in a stressful environment.
Ability to create a positive patient experience for each patient.
Ability to work with other staff members effectively as part of a team approach to their care.
Three years’ clinical experience required. Certified Medical Assistant, LPN, Radiology Technologist, Certified Athletic Trainer or related field with appropriate credentials based on role required. Graduate of a CAAHEP accredited medical assisting program with a current CMA credential or a graduate from ABHES-accredited medical assisting program with a current RMA credential required for CMA. NC License required for LPN. ARRT certification required for Radiology Technologist. Orthopedic experience preferred.
