Job Overview

We are seeking a highly organized and compassionate Practice Care Coordinator to join our team at Insite Health. The Practice Care Coordinator will be responsible for coordinating care for patients with mental health conditions, working closely with healthcare professionals to ensure that patients receive the best possible care. The successful candidate will be passionate about mental health and have experience in healthcare coordination and patient advocacy.

Key Responsibilities:

  • Coordinate care for patients with mental health conditions, working closely with healthcare professionals, including psychiatrists, therapists, and social workers, to ensure that patients receive appropriate treatment
  • Communicate with patients and their families to provide education and support, and to ensure that they understand their treatment plans
  • Facilitate referrals to other healthcare professionals and community resources, such as support groups or financial assistance programs
  • Develop and implement care plans for patients, in collaboration with healthcare professionals, to ensure that treatment goals are met
  • Monitor patient progress and adjust care plans as necessary, based on patient response and feedback from healthcare professionals
  • Maintain accurate and up-to-date patient records and communicate relevant information to healthcare professionals as needed
  • Advocate for patients and their families, ensuring that their rights and preferences are respected and that they receive the best possible care
  • Collaborate with other departments within the company, such as customer service and billing, to ensure a cohesive approach to patient care
  • Keep abreast of industry trends, regulations, and best practices related to mental health care coordination

Requirements:

  • Bachelor's degree in nursing, social work, psychology, or a related field
  • Minimum of 2-3 years of experience in healthcare coordination and patient advocacy, preferably in mental health
  • Excellent communication and interpersonal skills to effectively advocate for patients and collaborate with healthcare professionals
  • Strong organizational and time-management skills to manage multiple patient cases and prioritize competing demands
  • Ability to work independently and as part of a team to ensure high-quality patient care
  • Experience using electronic health records (EHRs) and other healthcare software to manage patient records and coordinate care
  • Knowledge of mental health conditions and treatments, and an understanding of the challenges faced by patients with mental health conditions
  • Ability to maintain confidentiality and adhere to ethical and professional standards of practice
