Patient Care Navigators:
Are you exceedingly driven, dedicated, and passionate about caring for your patients? • Our Patient Navigators perform the following:
• Patient focused progression of care by supporting GSUH Departments as needed
• Offering to call a family or friend to notify them of the admission
• Assisting in addressing Food insecurity needs
• Scheduling and Confirmation of Discharge Appointments
• Assisting with patient movement across organization
• Answering and Triaging of phone calls

• What you’ll bring:
• Passion for Patient Care and Patient Experience
• Certification Basic Life Support (BLS)
• High School Diploma or GED Equivalent
• LPN or CNA background preferred
