Operation Mend is seeking to hire a full time Patient Navigator. Under the direction of the Program Director and supervision of the Patient Coordinator Supervisor, You will provide high touch navigation, advocacy and administrative tasks for patients and family members on and off the UCLA Campus as designated by the Operation Mend program. You will work independently and with a team to complete the following duties: * Coordinate all travel, hotels, and accommodations using the UCLA BruinBuy, Concur and Salesforce platforms for Operation Mend patients and family members. * Escort patients and family members to and from the airport, clinical appointments, surgeries, and procedures events, and other locations in Westwood, Santa Monica and other UCLA clinic sites. * Be responsible for administrative tasks that include appointment summaries, maintaining up to date written standard operating procedures for the procurement of office supplies, use of UCLA platforms, patient template emails, Infection Prevention knowledge of current policy and hospital protocols, and other assigned administrative duties. * Must be flexible, take initiative, have strong communication skills, organized, reliable and excel in problem solving. * Be detail oriented and able to coordinate and manage special needs accommodations and requests for patients and family members. * Responsible for maintaining and delivering patient individual schedules related to patient care at UCLA. Individual will maintain the highest level of professionalism, accurate knowledge of the Operation Mend programs and be a representative in outreach settings, as needed. Pay Range: $22.03 - $43.68 hourly.
* Experience working in a health care system setting, recommended * Working knowledge of Concur, BruinBuy and Salesforce, preferred * Knowledge of the UCLA Health landscape and medical facilities, preferred. * Ability to establish and maintain cooperative working relationships with Operation Mend staff, medical directors and providers * Skill in listening and comprehending information received in person or over the telephone to navigate patients. * Ability to manage specific needs for individual patients and family members * Detail oriented and ability to communicate accurate information both verbally and in written form * Ability to work standing, walking, and sitting for extended periods of time * Availability to work evenings and/or weekends * Interpersonal skills to interact diplomatically and sensitively with patients, staff, and visitors * Working knowledge of Microsoft Office Suite programs (Excel, Word, PowerPoint, etc.), strongly recommended * Ability to develop priorities, meet deadlines despite interruptions, and perform multiple complex assignments * Ability to work independently on assignments with minimal supervision * Ability to take direction and receive feedback
