Responsibilities

Franciscan Medical Group, a part of Virginia Mason Franciscan Health, currently has an opening for a grant funded Patient Relations Specialist position with our Franciscan Northwest WA Family Residency Clinic in Bremerton, WA. This position is Monday through Friday with no nights, weekends or organizationally recognized holidays required.

Job Summary:

This job is responsible for coordinating and tracking patients that receive a positive lab result for HIV, Hep C or Syphillus. They will be reaching out to these patients, families or anyone else involved in care coordination to help get them established with a primary care physician and receive the appropriate education and treatment.


Essential Duites:

  • Rounds on assigned facilities/departments with FOCUS grant team to assure patients/families’ questions and concerns are answered; resolves, or recommends resolution of, issues within defined scope of position and in accordance with established procedures.
  • Maintains contact with patient and/or appropriate caregivers.
  • Documents and tracks all communications with patient and or appropriate caregiver.
  • May serve as a centralized point-of-contact for Patient and/or Caregiver
  • Cultural Sensitivity and Competence: Demonstrates proper use of communication tools/materials for effective communication and understands how the culture(s) of patient populations can affect communication, collaboration and the provision of care, treatment and services. Patient Population Served: Demonstrates knowledge and proper skills associated with the department’s defined specific populations served.
  • Performs related duties as required.

Qualifications

Education/Work Experience
:
  • Four years of recent customer service work experience in a clinical environment or business setting that demonstrates attainment of the requisite job knowledge/abilities.
  • Associate’s degree in related field preferred.
