General Purpose of Position:

  • The Certified Nursing Assistant (CNA) serves as an aide to the RN or LPN by performing regular assignments and/or delegating responsibilities under the direction of the nurse or provider. This team member is responsible for providing patient care at a proven level of safety, depending on his or her scope of practice and degree of competence.
  • The Personal Care Assistant (PCA) is responsible for the client’s personal care needs and surrounding and may supply temporary relief (respite) for the client’s primary caregiver. The PCA provides care in the home, in the hospital, or in a long-term care facility (nursing facility). The PCA is also referred to as a nursing assistant, respite care worker, or personal support aide.

Primary Responsibilities:


Personal Care Assistant (PCA)

  • Activities of daily living assistance including care needs, meal preparation, and assistance with eating. Personal care needs may also include bathing or assistance with bathing, dressing, grooming, toileting, ambulating, and transferring from bed to chair or other locations.
  • Routine housekeeping tasks including laundry, changing bed linens, dusting, washing dishes, vacuuming, and other light household responsibilities.
  • Transporting and accompanying patients to doctor’s office appointments when necessary.
  • Errands on behalf of the client as needed and directed by the supervisor.
  • Serve as a companion for the client and/or provide temporary relief to the caregiver.
  • Provision of specialized client care as instructed by the supervisor and as evidenced by documentation of training.
  • Communicate to CHC the client’s needs and any changes in the client’s status through written documentation and verbal communication.
  • Remain current in healthcare trends – obtain practice management certifications, maintain professional organization membership, and attend continuing education courses/webinars as needed.
  • Show patience and respect in caring for the sick, elderly and disabled clients.
  • Work under close supervision of agency staff and cooperate with the client’s family and staff from other agencies involved with the client’s care.
  • All other tasks as assigned

Certified Nursing Assistant (CNA)

  • Patient Care
    • Prioritizes and administers basic patient care, such as toileting, bathing, linen and clothing changes, fresh water and ice, ambulation, comfort measures, and other patient requirements.
    • Transporting and accompanying patients to doctor’s office appointments when necessary.
    • When responding to patient and family demands, it provides superior customer service.
    • Errands on behalf of the client as needed and directed by the supervisor.
    • Serve as a companion for the client and/or provide temporary relief to the caregiver.
    • Provision of specialized client care as instructed by the supervisor and as evidenced by documentation of training.
    • Communicate to CHC the client’s needs and any changes in the client’s status through written documentation and verbal communication.
    • Remain current in healthcare trends – obtain practice management certifications, maintain professional organization membership, and attend continuing education courses/webinars as needed.
    • Show patience and respect in caring for the sick, elderly and disabled clients.
    • Work under close supervision of agency staff and cooperate with the client’s family and staff from other agencies involved with the client’s care.
    • All other tasks as assigned.
    • Lead patient developmental activities. Supervise children at play.
  • Collaboration:
    • Collaborates and consults with the RN and other multidisciplinary team members to provide optimal patient care.
    • Exhibits proficiency in collaboration, prioritizing, communication, and organization.
    • As directed, implements the care plan.
    • Performs responsibilities allocated within the Scope of Practice. Performs specialized processes using validated expertise and in accordance with regulatory requirements.
    • Promotes a safe environment by adhering to the safety policy and responding appropriately to an emergency.


Education, Skills and Experience Requirements:

  • Minimum 1-2 years of CNA/PCA experience
  • CNA: Certification as a CNA in the State of Georgia and registered through the Georgia State Registry
  • PCA: Successful completion of a health care or personal care credentialing program or willingness to undergo completion of training and PCA exam.
  • Maintain CPR and First Aid certification, auto insurance and annual TB screening

Physical Requirements and Work Environment:

  • Environment: Work is performed primarily in a home setting with extensive patient contact. Due to the nature of the work, the LPN may be exposed to bodily fluids and odors on an occasional basis.
  • Physical: Primary functions require sufficient physical ability and mobility to work with a patient; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, carry, push, and/or pull light to moderate amounts of weight (up to 30 lbs); and to verbally communicate to exchange information.
  • Vision: Close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
  • Hearing: Hear in a normal audio range, with or without correction.
