JOB SUMMARY

  • THIS POSITION IS LOCATED IN PAGE, ARIZONA.
The Peer Support Specialists (PSS) assigned to PCMH/OTP Community-Based Services will perform only the duties of a Peer Support Specialist addresses one-on-one support needs and provides support for day-to-day problem-solving related to normalization and reintegration into the community. The Peer Support Specialist utilizes best practices of assertive engagement, harm reduction and peer support when working to engage with members in the community and the facility. The Peer Support Specialist embraces the recovery philosophy and promotes a safe and inclusive recovery environment for clients.

ESSENTIAL JOB DUTIES OR RESPONSIBILITIES
  • Coordinates patient care with the CBI Care Team, as well as other CBI facilities and staff.
  • Acts as first point of contact for patients beginning treatment at PCMH facilities.
  • Participates in Adult Recovery Team (ART) meetings and the development of treatment plans for patients.
  • Works in a variety of settings with culturally diverse individuals and communities and has the ability to be culturally sensitive and appropriate.
  • Shares knowledge and experience with patients, as appropriate.
  • Advocates for patient to access to services and resources in the community.
  • Facilitates two-way communication between patients and their Care Team.
  • Performs Community Based Outreach as needed in an effort to engage assigned patients and/or patients who miss scheduled sessions or appointments.
  • Serves as a liaison for family members and natural supports who have been identified by the patient; ensuring releases of information (ROI) are documented in the EHR.
  • Participates in case staffing with other care teams as necessary.
  • Demonstrates and maintains knowledge of relevant federal and state regulations pertaining to managed behavioral health and insurance products.
  • Utilizes motivational interviewing strategies to assist patients in increasing their insight and commitment to their recovery.
  • Works in coordination with the care team to ensure completion of the patient intake process into Outpatient.
  • Completes all error corrections identified by Medical Records personnel as outlined in CBI Policy and Procedure.
  • Ensures all activities are documented in the electronic medical record in accordance with CBI Policy and Procedures.
  • Completes and documents all patient interactions and progress notes within 24 hours, if unable to do so Practice Manager must be contacted immediately.
  • Participates in discharge planning from hospital and inpatient settings.
  • Other duties as assigned by supervisor and/or manager and/or administrative support.
SUPERVISORY RESPONSIBILITIES

This position has no supervisory responsibilities

MINIMUM EDUCATION & EXPERIENCE REQUIRED
  • Highschool diploma or GED required.
  • Must have lived experience in recovery from mental health and/or substance use disorders, homelessness, family members and veterans.
  • Minimum of six (6) months experience working in the Behavioral Health and/or Substance Abuse field required.
LICENSES, CERTIFICATIONS, CREDENTIALS, REGISTRATIONS
Current AZ Driver’s License (valid and in good standing).
Clear 39-month Motor Vehicle Record.
Certified Peer Support Specialist or able to obtain Peer Support certification within 90 days (3 Months).
