Position Title: Pharmacist
Department: Patient Care Medical Group
Reports to: Medical Director
Position Summary:

Essential Functions:
- Assesses the health and pharmaceutical needs of patients.
- Consults with patients and/or medical practitioners on drug indications/contraindications, dosage, drug interactions, and side effects; provides other drug and pharmaceutical information as appropriate.
- Provides education, guidance and training to patients, students, pharmacy interns, and or trainees during course of work and monitors performance.
- Monitor and evaluate the effects of pharmaceutical practices and plans, and adjusting if necessary.
- May take patient's histories as required; makes initial clinical assessments and refers patients to appropriate clinicians.
- Develop/provide educational trainings and materials to promote healthy lifestyle choices and healthy medication habits.
- Follow the latest research in geriatric pharmaceuticals, and participating in it.
- Performs routine quality control testing of equipment, environment and record keeping as required by federal, state and local regulations.
- Assist the patient in their counseling needs and maintaining their morale.
- Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
- Work with interdisciplinary team to provide appropriate pharmaceutical education, management, and referrals.
- Enter patient information into the appropriate database.
- Offering patients pharmaceutical education to improve their health
- Presenting facts on pharmaceuticals and management at community events and speaking opportunities.
- Engaging in pharmacy counseling to help patients develop and implement medication management plans Job
- Talking to patient’s primary care physician or other health care professionals about the pharmaceutical needs of the patient
- Guiding patients and their families regarding medication management.
- Creating research reports and other publications that document and communicate research results.
- Performs miscellaneous job-related duties as assigned.
Qualifications:
Bachelor's degree in Pharmacy or Pharm.D.; at least 6 months of experience directly related to the duties and responsibilities specified. Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. Additional certifications and state licenses may be required. At least 1 year of geriatric experience.
