Prism Performance & Physical Therapy is a small, privately owned physical therapy company offering orthopedic physical therapy services, as well as sports performance and injury prevention training.

Prism is expanding our team to include an additional PT and PTA. Interest and/or experience with athletes and wellness is encouraged. We are continually seeking creative methods to connect with and empower our patients and athletes to live their best lives.

Duties & Resposiblities:

* Direct Patient Care & Planning

* Communication with Referring Physician & Team

* Daily Documentation, Progress Notes & Discharge Summaries

* Proactive Planning & Implentation of Systems

Qualifications:

* Current IL Licensure *(New Grads Considered)

* Current CPR Certification

* Willingness to Travel Between Lockport & Manhattan Facilities

Job Types: Full-time, Part-time

Pay: $28.00 - $55.00 per hour
