The Mary T Family of Companies is seeking a full-time Outpatient Physical Therapist to join our team at our new Mary T Outpatient and Wellness Clinic. Our outpatient clinic is located in Coon Rapids, Minnesota and will be opening this spring!

This will be a full-time position with on-location daytime hours Monday - Friday.

ESSENTIAL FUNCTIONS AND BASIC JOB DUTIES

• Evaluate the physical needs of a client per physician’s referral, and establish a plan of care that is effective, client-centered, and within their scope of practice.
• Obtains physician’s orders, authorization for treatment and reimbursement as necessary.
• Establishes and maintains therapeutic relationships with clients and caregivers.
• Has a clear understanding of state, federal, and professional standards of practice, and adheres to these standards in the performance of their services.
• Ensures documentation is completed according to state, federal, and professional regulations and standards of practice, and as outlined in the Mary T documentation policy and procedures.
• Ensures documentation reflects client progress as related to treatment goals.
• Modifies plan of care and grades treatment techniques and environment to support maximal functional independence and progress toward goals.
• Provides a clear and reasonable action plan for discharge from the start of care, ensuring that client and caregiver education is provided and updated frequently to maximize outcome of therapeutic services.
• Collaborates with the client’s care team in promoting a home environment which will support the follow through of home programming and recommendations for maximal independent functioning
• Supervision of PT Assistants and caseloads

BASIC QUALIFICATIONS FOR THIS POSITION

EDUCATION:

• Masters or Doctorate degree in physical therapy from an accredited school of physical therapy.

REQUIRED CERTIFICATION/LICENSURE:

• Licensed in the state of MN.
• BLS and CPR certified.

PREFERRED EXPERIENCE:

• A minimum of 1 year of experience in an outpatient setting is preferred.
• Experience with individuals with developmental disabilities is preferred.
• Experience in home care therapy is a plus.

KNOWLEDGE, SKILLS, ABILITIES:

• Demonstrates and practices the principles of proper body mechanics for all physical activities
• Practices universal precautions, standard and facility infection control procedures
• Proficiency in computer skills for managing electronic medical records, and ability to learn proficiency with company software data systems.
• Demonstrates flexibility and creativity in patient care, time management, and the ever-changing world of healthcare.
• Implements evidence-based practices in evaluation, treatment planning and providing therapeutic services.
• Demonstrates initiative to further their skills and knowledge in the area of physical therapy and shares best practices with colleagues.
• Demonstrates exceptional verbal, written and interpersonal communication skills.
• Collaborates with colleagues in a kind and professional manner.
