Our Community-Based Services provide one-to-one health care for seniors living throughout the Puget Sound region. Services are customized and provided by professionals when and where you need them most, often in the comfort of your home. It’s all part of our Continuum of Care – and caring – that is never-ending.

Whether recovering from illness, surgery, an accident or living with a chronic condition, clients receive assistance at home from our HOME HEALTH nurses, therapists, social workers and other skilled caregivers who help maximize health, function, and independence.

We are seeking immediate placement for:

Signing Bonus: $10,000 !

Part Time or PRN
Physical Therapist

• 
  See no more than 5 patients a day!
• 1-on-1 care only; no Group Therapy sessions!

Territory: Eastside (northern area of the Eastside) King County

Key Responsibilities:

• Assess functional status (strength, mobility, gait, ROM), potential for rehab, and evaluate home environment/DME needs
• 
  Comfortable communicating with providers, clinical colleagues and management
• Develop a care plan in collaboration with community providers, and deliver physical therapy services accordingly

• Coordinate and communicate patient progress and/or changes in condition to overseeing physicians and interdisciplinary team members
• Prepare and submit documentation of visits and progress towards goals

Applicants must have:

• An active PT license in WA State
• A reliable car with current insurance coverage and a current driver's license
• Experience working in a Medicare certified Home Health agency
• 
  2-3 days per week with flexibility
• Organized and self starting to manage caseload/scheduling independently
