Compensation Range: $58,000 - $63,000 annually commensurate with experience
Overview:
Program Coordinator will be provided oversight and coordination to the program to increase access and promote linkage to individuals new to care, at risk for being lost to care and those not currently adherent to ART. The coordinator with provide services for Data to Care and Retention and Adherence Program. This individual will be responsible to ensure coordination with other programs and will be responsible for the supervision of the Outreach Specialist and peers for the DTC program.
Responsibilities:
- Responsible for day-to-day oversight of program deliverables and program staff.
- Responsible for coordinating and developing partnership with internal and external programs to increase access to potential program participants.
- Maintain active caseload and determine referral for home visit/escort to appointment based on patient needs.
- Responsible for initiating intake and assessment process; ensures that descriptive information is provided to applicants and agencies; ensures that inquiries about the application and admissions process are responded to in a timely and appropriate manner.
- Responsible for performing eligibility review, screenings, risk assessments, and make referrals when necessary.
- Provide on-going supervision to the outreach specialist and peer stipend workers.
- Coordinate and plan all outreach activities as required by the scope of service.
- Responsible for the preparation and submission of monthly narratives and data entry as required by program standards and guidelines.
- Participates in morning huddles.
- Participates in primary care interdisciplinary meetings.
- Will led quality projects for DTC.
- If necessary, assists clients in applying for entitlements or refers them to appropriate governmental agency and advocates on client’s behalf to obtain the necessary entitlements.
- Participate in the provision of health education and psycho-educational sessions with clients.
- Provides training to staff as it relates to program standards.
- Functions as primary liaison between program and outside providers (e.g., hospitals, other programs and social service agencies).
- Performs other duties as required by management staff.
- Supports and participates in advocacy efforts to end the twin crises of homelessness and AIDS.
- Master of Public Health (MPH), Master Social Worker (MSW), or equivalent with at least two years of patient navigation experience; or Bachelor of Arts (BA)/Bachelor of Science (BS) with at least five years of experience of patient navigation experience, and at least two years of experience managing services for LGBTQIA+ people with HIV.
- Experience in a health care setting is highly desirable.
- Knowledge of HIPAA regulations are required.
- Strong organizational and clinical skills required.
- Candidate must be willing to work on weekends.
