Silicon Valley Medical Development is currently seeking a talented Patient Care Representative to join our growing healthcare team!

Pay: $130,000-$165,000

Location: Los Gatos, CA

Silicon Valley Medical Development, (SVMD) LLC is a growing provider organization with community-based ambulatory care clinics and medical practices in Santa Clara County.

The Quality Improvement RN actively participates in the development and measurement of quality improvement programs and processes to determine if programs have a reasonable clinical basis and provides input from clinical and regulatory experience. The Quality Improvement RN monitors member health outcomes through internal and external data collection (Potential Quality Issues, Healthcare Effectiveness Data and Information Set (HEDIS), Performance Improvement Projects, etc.) to determine effectiveness in maintaining safe care environments that meet regulatory requirements and to ensure members are receiving care that meets evidence based standards. The Quality Improvement Nurse works closely with the Quality Committee and the Medical Director.

Qualifications/Licensure/Education:

  • Bachelor of Science in Nursing required
  • Master of Science in Nursing preferred
  • Current RN License
  • Minimum of 3 years of clinical experience required; at least 2 years in an ambulatory care, medical foundation, medical group organization or care setting.
  • Previous experience in quality improvement methodology (PDSA, Lean, Six Sigma) and/or accreditation strongly preferred.
  • Epic EMR experience preferred

Knowledge/Skills/ Abilities:

  • Ability to accurately type a minimum of 40 WPM
  • Strong verbal and written communications, and active listening skills
  • Solid patient/guest relations skills with the ability to communicate in a professional, courteous and efficient manner
  • Ability to perform basic math
  • Excellent attention to detail
  • Ability to effectively organize and prioritize task in order to complete assignments within the time allotted to maintain standard workflow
  • Ability to work effectively as a team member who is flexible, cooperative and willing to assist other

Location: Silicon Valley Medical Development ยท SVMD Operations
Schedule: Full Time, Days, 8:00 a.m. - 5:00 p.m.
