Here at Physician HealthCare Network, we are one of the leading healthcare providers in St. Clair County. We're proud of the care we provide to our patients. We are seeking to hire an individual for our Physician HealthCare Network's Administration Building.

GENERAL SUMMARY OF DUTIES: The Quality Outreach employee is required to be a Registered Medical Assistant (RMA) and assists with and promotes continuity of quality care and provides overall support to the Quality Department. This position is responsible for making outreach calls to patients and assist with various outreach campaigns in an effort to increase patient volumes. Provides support to the team to facilitate the achievement of the program’s goals. Candidate must be comfortable making phone calls and speaking to patients over the phone.

The Quality Outreach employee will also process TCM/Transitional Care Management calls to patients as they are discharged from the area Hospitals. The patient call will include a review of the patients comfort level with their discharge plan and schedule the patient to see their PCP and specialists as directed. A medication reconciliation will be completed to ensure the clinic chart includes the changes to medications made during the hospital stay and that the patient understands those changes.

ESSENTIAL FUNCTIONS:

· Providing ongoing, active outreach to patients

· Contact patients with identified gaps in care and communicate recommended tests/services to the patient and educates patients regarding their condition

· Support the existing PCMH (Patient Centered Medical Home) foundation in our state

· Provide support for Pay-for-Performance quality programs for the practice

· Retrieve, interpret, and transmit data captured as it relates to quality measures

· Manage call list of patients who are out of compliance with chronic conditions, wellness and preventative visits

· Track all booked and arrived appointments for patients generated through outreach efforts

· Manages follow up bucket for missing/untied orders

· Assist with generating recall letters and making appointments for various areas of the practice

EXPERIENCE:

. Experience working as a Medical Assistant.

· Two year work experience in health care organization preferred

(Knowledge of ICD-10 and CPT Coding preferred but not required)

ABILITIES:

- Ability to document with detail and accuracy all communications within the patient record.

- Provide education in support of testing for chronic conditions and/or prevention.

- Confident dealing with patients over the phone, including patients who are not happy.

- Ability to multitask – work on multiple projects in the same day, may be interrupted frequently, makes good use of time

-Comfortable speaking on the phone/good phone etiquette in a variety of situations

- Problem-solving skills - knows where to find answers on their own and knows when it is appropriate to ask for help

- Comfortable with computer – accurate typing, using two screens, switching between multiple tabs, use of email, Microsoft Word, Microsoft Excel, etc.

Spelling/grammar at a professional level

EDUCATION:

· High School Diploma or Equivalent

. Registered Medical Assistant (RMA)

Job Type: Full-time

Pay: From $16.00 per hour
