Referral Coordinator
Job description
POSITION/JOB SUMMARY:
The Referral Coordinator will work as an educator, healthcare resource, and patient advocate. Additionally, the Referral Coordinator will focus first and foremost on the patient’s best health outcomes.
SUPERVISORY RESPONSIBILITIES:
- None
KEY RESPONSIBILITIES & DUTIES:
- Effectively communicate with patients to ensure follow-through with referrals via phone call backs, and returning voicemail.
- Coordinate and update patient charts through EMR with physician orders and communicates with patient.
- Refers patients to external doctors/specialists and communicates the clinician’s recommendations.
- Responds to questions in a timely manner and explains the rationale behind the planned treatment, and coordinates care across the healthcare continuum.
- Provide clinical resources- where to seek a second opinion, treatment, or testing that may not be available.
- Answers questions as appropriate and serve as liaison to the physician.
- Support timely scheduling of appointments, diagnostic testing, and procedures.
- Must have strong communication skills in order to meet patients’ needs.
- Maintain strict patient confidentiality in compliance with HIPAA.
- Other duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES:
- Must be able to communicate effectively in English and Spanish, both verbally and in writing.
- Additional languages preferred.
- Knowledge of medical terminology required.
- Must be computer literate (e.g. basic software and Microsoft Word).
- Excellent telephone etiquette. Ability to speak clearly and concisely.
- Must be detail oriented.
CREDENTIALS / EDUCATION / EXPERIENCE:
- High school diploma or general education degree (“GED”) required.
- At least 1-2 years of experience in the medical field, preferably experience in utilization management, case management, care coordinator, or extensive knowledge of hospital systems.
