This is a full time position working day shift in our Skilled Nursing unit at Oro Valley Hospital. The position is 8am - 5pm and requires 1-2 weekends per month.

Work with case management, nurses, physicians, therapists, and executive director to admit patients to the Oro Valley Skilled Nursing Unit. Serve as a part of the discharge planning team, assessing patients for the appropriate level of care at discharge from Northwest Medical Center and Oro Valley hospital. Serve as a part of the overall admissions team, including other liaisons, the rehab director, executive director, therapy and nursing supervisors, and rehab physicians. Communicate with physician and charge nurses regarding special considerations for new admissions to ensure continuity of care. Educate patients and family members regarding post acute services and appropriate levels of care.

Our Skilled Nursing unit is located inside Oro Valley Hospital. Offering 33 private rooms, patients can comfortably recover from a variety of illnesses and injuries. Our goal is to help patients return to their lives outside the hospital. As a nurse on our team, you will work with other health care professionals while providing quality, patient centered-care.
