Job Summary:
We are seeking a highly-motivated and talented individual to fill the position of Registered Nurse Care Manager RN in Rio Rancho, NM. As a member of our healthcare team, the RN Care Manager will work closely with other professionals to coordinate patient care, develop care plans, and monitor patient progress.
Responsibilities and Duties:
- Assess patient needs and develop individualized care plans
- Collaborate with other healthcare professionals to coordinate patient care
- Monitor and evaluate patient progress, and adjust care plans as needed
- Educate patients and their families on health-related issues and self-care techniques
- Ensure that all patient care documentation is accurate, complete, and up-to-date
- Maintain a safe and therapeutic environment for patients and staff
Qualifications and Skills:
- Graduate of an accredited nursing program
- Current RN license in the State of New Mexico
- Minimum of two years of nursing experience, preferably in care management or case management
- Strong communication and interpersonal skills
- Knowledge of healthcare regulations and standards
- Computer literacy and experience with electronic health records
