PRIMARY ACCOUNTABILITY: The Centralized RN Case Manager’s primary responsibility is to provide medical support to the Clinicians & Medical teams. Works with the medical team to provide quality, comprehensive care to patients with complex health situations. Duties of the RN Case Manager include individual screening, chronic condition(s) education, medical assessments, charting, care planning, stabilization, observation and maintenance, ongoing coordination of follow-up care, and support for an individual’s self-management.

MUST HAVE: WA State RN license/certification
