Faith Home Healthcare, INC. is based in Yucca Valley, California. We service the entire California Region. As we are growing, we are looking for motivated and enthusiastic Registered Nurses/ Licensed Vocational Nurses to join our Home Health team. Under general supervision, this position is assigned to provide skilled nursing care to patients in their homes, following the orders of the attending physician, and functioning as a member of the interdisciplinary team.

Qualifications:
Minimum Education:
Minimum Education:
- Associate Degree in Vocational Nursing/ Nursing or Degree in Nursing from an accredited school.
Minimum Experience:
- 1 year of experience as an LVN/RN, preferably in home healthcare.
Required Certification/Registration:
- CPR Certification.
- California RN license.
- Valid CA Driver’s license.
- Current automobile insurance.
Preferred Courses/Training:
- Courses in Home Health and/or Hospice.
- Public Health Nurse Training.
Skills: Administrative:
- Prior supervisory experience of Aides preferred.
Skills: Machine:
- Knowledge of the use of Personal Computers.
Physical Demands:
- Considerable reaching, stooping, bending, kneeling, and crouching.
- Lifts supplies/equipment *Lifts, positions, pushes, and/or transfers patients
- Lifting requirement 25 lbs+
- Prolonged, extensive, or considerable standing/walking.
Primary Duties:
- Evaluate and assess the patient and his/her home environment.
- Communicate with the physician and other team members to develop and coordinate the care plan and to report any changes in the patient’s condition.
- Assist in implementing the patient’s care plan.
- Maintain current, accurate documentation in the client’s medical record.
- Maintain established departmental policies and procedures, objectives, quality improvement program, and safety, environmental, and infection control standards.
- Maintain health care instruction to the patient as appropriate.
- Serve as the liaison between the patient, support staff, and physician.
- Enhance professional growth and development through participation in education programs, current literature, in-service meetings, and workshops about home care.
- Attend a required number of departmental meetings per year.
- Direct and supervise home health aides.
- Demonstrates sensitivity and competence in the care of ill adults and patients relative to determining each patient’s age-related physical, psychological and cultural needs.
