Comfort Home Care LLC is seeking candidates for a Client Service Coordinator Registered Nurse (RN) position in District of Columbia. This is a PRN position with a competitive yearly rate. Since this is a part time position, there are no benefits associated.
Role Overview
The Client Service Coordinator (CSC) is responsible for overseeing all client care. The CSC works directly with client families to create a care plan suited to the client’s needs and instructs and directs the caregivers on how to work with each client. In addition, the CSC ensures that Comfort Home Care complies with all clinical regulations set forth by the Washington D.C. Department of Health. This position also supports clients on an ongoing basis, consulting and/or visiting them to assist with their needs.
Goals
To complete the new client intake process within 48 hours. To respond in a timely manner when contacted by CHC staff members to schedule the assessments – time is of the essence. To follow all clinical regulations set forth by the District of Columbia.
Metrics
All assessments are within the 48-hour time frame.
The family/client knows your name and is given the arrival date and time for home assessment
Caregivers are completely aware of care plans for each client and implementing current are plans Regular contact with clients to monitor health and care, and to answer questions
Job requirements
1. Intake assessments for new clients
2. Writing care plans for Comfort Home Care, families and caregivers
3. Meeting with aides to go over care plans
4. Ongoing assessments every 90 days or as needed with clients
5. Speaking with clients when issues arise
6. Skills assessments for new caregivers
7. Regular client care phone calls
8. Must be flexible and willing to accommodate the 48-hr requirement set forth by DC DOH
9. Position involves travel to clients’ homes in DC
10. Other duties as assigned
Education
Bachelor’s degree in nursing
Experience
2 years’ experience as an RN
Familiarity with home care desirable
Aptitude

Skills
Current Registered Nurse license in Washington D.C.
Driver’s license and vehicle Microsoft Office Suite
