Job SummaryWe are seeking a Home Health Registered Nurse to join our team. In this role, you will primarily be creating and managing plans of care for individual patients in their homes.

Responsibilities
  • Coordinate to assess, plan, implement, or evaluate patient care plans in patient home
  • Administer medications and monitor patients for side effects or reactions
  • Document any changes in patient conditions and report on changes
  • Maintain accurate records of patient care and discuss observations with Nurse Supervisor
Qualifications
  • Active PA Registered Nurse (RN) license
  • Previous experience as a Registered Nurse in Home Health is preferred
  • Valid driver’s license
  • First aid/CPR certification required
