We are hiring IMMEDIATELY for a Remote Licensed Clinical Social Worker to join one of our community mental health providers!

Summary:

• Document services provided and communicate with client and care team as needed.
• Assess and triage individuals in crisis. Provide short-term counseling.
• Prepare documents for referrals if needed.
• Coordinate care with medical providers, case managers, jail personnel community agencies both for individuals and for population health improvement.
• Gambling services, including: education; assessment; diagnosis; treatment planning; counseling and discharge planning,
• Pre-commitment investigations.
• Court liaison as related to Aid & Assist, 370 population, Hope, Mental Health and Drug Court.
• Addiction services including: education; assessment; diagnosis; treatment planning; counseling and discharge planning.
• Multi-disciplinary, community-based, team approach to provide services using Assertive Community Treatment (ACT), including education, assessment, diagnosis, treatment planning, counseling and discharge planning.
• Multidisciplinary team approach for Early Assessment Support Alliance (EASA) including: education; assessment; diagnosis; treatment planning; counseling and discharge planning.
• Parent Child Interaction Therapy (PCIT).

Pay: From $87,000.00 per year
