Position Summary

The Case Manager collaborates with the medical staff, interdisciplinary team, and external resources to assess, coordinate, implement, and evaluate services required to promote quality outcomes and efficient resource management. Follow up and patient referrals and performs clinical assessments to ensure appropriate admissions to the facility.

Qualifications

• Required
  • BSN or above required or currently enrolled in BSN program
  • Maintains current Registered Nurse license from the Nevada State Board of Nursing
  • Current American Heart Association (AHA) Basic Life Support for Healthcare Workers
  • Minimum two (2) years clinical nursing experience in an acute care hospital
• Preferred
  • Utilization management/quality management experience
  • Case management certification
