Job Summary and Qualifications
We are seeking a skilled Case Manager Registered Nurse (RN) to provide Utilization Review and Case Management functions for the entire facility. Your primary charge is to identify and coordinate treatment plans for the patient while in the hospital and to identify the best healthcare regime for the patient upon discharge. In this role, you must maintain strong relationships with all parties and be a strong advocate for the patient, providing skilled professional guidance using your awareness, critical thinking skills and best practice methodologies while adhering to all regulatory compliances. - In this role, you will perform preadmission, concurrent and retrospective review for all payers.
- Work closely with physicians, MD office and ED, applying appropriateness criteria to all new admissions
- You will identify and intervene coordination of care on patients, including length of stay and resource utilization issues
- You will provide assessments for patients/families having complex needs and/or medically complex diagnoses
- Collaborate with clinical staff to develops treatment goals and plans interventions to assist the patient and family
- You will refer patients requiring extended care post discharge to long term care, skilled nursing and/or acute rehab facilities and arrange for transfer
- You will work closely with community agencies to assist patients and families after discharge
- Be aware of current trends that influence health care practice and assures compliance with regulatory, governmental and organizational standards
REQUIREMENTS
- Bachelor of Science in Nursing (BSN) preferred.
- Registered Nurse (RN) License in NH or valid Compact State License
- BLS (American Heart Association BLS Provider or Red Cross BLS)
- One to three years of nursing case management experience preferred
