JOB FUNCTIONS/RESPONSIBILITIES
- Receives case referrals.
- Reviews available patient information related to the case, including disciplines required, to determine home care needs.
- Reviews patient’s clinical diagnosis, medications, procedures, and clinical course.
- Assists care coordinators in establishing immediate and long-term therapeutic goals, setting priorities, and developing plans of care.
- Attends case conference meetings with organization personnel to facilitate coordination of care.
- Conducts record reviews and communicates findings and recommendations to Clinical Director and appropriate organization personnel.
- Assists the Care Coordination team with the follow-ups, notes, and supply orders.
- Assists with staffing whenever needed.
- Complies with accepted professional standards and principles.
- Performs other duties and activities as delegated by the Clinical Director and Case Manager.
POSITION QUALIFICATIONS:
- A nurse licensed in CA is a plus.
- Knowledge and expertise in clinical decision-making.
- Complies with accepted professional standards and practice.
- Demonstrates excellent observation, verbal and written communication skills.
