Hospice RN On-Call
Location: Princeton, NJ (Middlesex Mercer Counties)
Shift: Full-Time
Hours: The On-Call Registered Nurse (RN) must be available to be on-call 5 pm to 9 am Mondays-Thursdays; and Friday 5 PM until Monday at 9 AM (7 days on/ 7 days off).
Salary: $78,000-$88,000 depending on experience
Requirements:
- Graduate of an accredited registered nursing program.
- Bachelors degree in Nursing preferred.
- Certified Hospice and Palliative Nurse (CHPN) certification preferred.
- Has a minimum of one (1) year general nursing preferred with current hospice, home health, medical, surgical, or critical care experience.
- Knowledge of pain and symptom management interventions preferred.
- Possesses understanding of Hospice philosophy and principles of death/dying.
- Registered Nurse licensed by the New Jersey State Board of Nursing in accordance with N.J.S.A. 45:11-23 et seq. and N.J.A.C 13:37.
- Valid drivers license and reliable transportation insured in accordance with New Jersey state requirements.
- Possesses satisfactory references from employers and/or professional peers.
- Passes satisfactory criminal background check
Responsibilities:
- Assume primary responsibility for the census during the assigned shift.
- Answer all phone calls received on behalf of the agency and follow up according to agency policies and procedures.
- Provide skilled nursing, intervention, and supportive care to patients in the facility and/or home setting as prescribed by the physician and in compliance with the Hospice Medicare
- Conditions of Participation, State licensure laws, accrediting body standards and agency policies and procedures.
- Administer medications in accordance with all Federal and State laws and rules.
- Accept responsibility of assignment to perform a specialized procedure only when qualified with specialty training, proven competency, clinical background and expertise in the specialty area.
- Assess the dietary/nutritional needs of Hospice patients and provide appropriate interventions relative to nutritional intake as the patients condition progresses.
- Works in cooperation with family/caregiver and IDT members to meet the physical and emotional needs of the patient/family/caregiver.
- Facilitate patient and family education to promote continuity of care and optimal patient/family outcomes for quality of life as defined by the patient/family/caregiver.
- Educate primary caregivers, volunteers, and employed caregivers to provide care for patients as indicated.
- Maintain regular communication with agency staff and the patients referring physician and/or case manager concerning patient/family care needs and response to interventions.
- Maintain up-to-date patient records so that problems, goals and interventions are accurately and clearly stated and changes are reflected as they occur.
- Complete documentation within agency guidelines and to meet regulatory requirements.
- Submit completed documentation within one business day after on-call services are provided.
- Accept referrals for new patients in accordance with the agencys policies and procedures.
- Communicate regularly with agency clinical staff to review problems or unique issues from on-call hours.
- Functions without supervision, utilizing time constructively and organizing assignments for maximum productivity.
- Participate in the agencys orientation and in-service training programs for professional staff.
- Participate in the agency's performance improvement activities and marketing and community programs as requested, to promote professional growth and understanding of hospice care.
- Demonstrates the highest degree of customer awareness by seeking opportunities to identify and meet the needs of internal and external customers.
- Is receptive to questions and criticism, and willing to offer assistance.
- Maintains accountability for patient satisfaction throughout the episode of care.
