POSITION TITLE: RN - Transgender Care Centralized Follow-Up Nurse
STATUS: Full Time , Non Exempt
REPORTS TO: Director of Centralized Follow-Up/ Director Transgender Care Program
COMPENSATION: $27 TO $35 an hour

PPOSITION SUMMARY
The Transgender Care Centralized Follow-Up Nurse will specialize in the case management and clinical follow-up of our transgender and gender expansive patients. They will be a core member of the Transforming Community Transforming Care (TC2) team as well as supportive staff to the larger clinical and administrative centralized follow-up team. Primary responsibilities include initial assessments and introduction of program to new patients, review of laboratory work, coordination with patient navigators and other TC2 team members, referral of patients for non-PPSLR services, and collaboration with clinical providers for ongoing clinical care. Must be organized, with the ability to multi-task and provide timely communication with CFU colleagues and health center staff.
PRIMARY DUTIES PERFORMED
Take patient calls regarding clinical medical questions, triage phone calls, work with patient navigator on patient needs, consult with the Director of the Transgender Care Program.
Perform initial assessment and introduction to the program, including review financial need and available resources.
Assess patient’s medical status and make appropriate decisions and actions.
Working under standing medication orders, provide treatments and refills as indicated in accordance with the PPSLRSWMO MS&Gs.
Responsible for managing gender expansive patient follow-up plans in collaboration with responsible clinician.
Participate in periodic TC2 team meetings.
With Patient Navigators, responsible for managing communication from patients on the Patient Portal.
Responsible for managing tasks assigned by providers.
Review and abstract medical records from outside facilities.
Complete medication prior authorizations as needed.
Make appropriate appointments in EPM for patients for treatment and follow-up of abnormal lab results, in-house referrals, and urgent ab follow up.
Monitor voicemail, return appropriate phone calls, and document client information and forward calls as appropriate.
Adhere to affiliate policies on professionalism, wait time on the phone, and the system for addressing client complaints.
Provide regular feedback to the Director of CFU and Director of TC2 regarding training and continuing education needs.
Participate in the affiliate efforts to achieve established productivity and revenue cycle goals.
Participate in affiliate efforts to improve the patient experience.
Perform other duties as required.
Clinical:
Assess patient’s medical history
Assess patient’s status and make appropriate decisions and actions
Assess patient vital signs
Document appropriately and thoroughly in EHR
Record usage and tracking of medications
Lab training and duties as needed
Patient education
Participate in the affiliate efforts to achieve established productivity and revenue cycle goals.
RISK MANAGEMENT AND QUALITY ASSURANCE
Adheres to PPSLRSWMO and department policies and medical standards and guidelines that ensure the provision of the highest quality and most professional services to all PPSLRSWMO clients
Protects PPSLRSWMO's assets from liability including but not limited to assuring proper informed consent has been obtained for any and all medical interventions requiring patient’s consent.
Assures for the physical safety of PPSLRSWMO clients, staff, and volunteers
Protects confidentiality of all PPSLRSWMO clients, donors, Board and all areas of operations
Follows HIPAA privacy and security guidelines
QUALIFICATIONS
Current Missouri and Illinois Nursing License. Must ensure that all licensure and certification are up to date and copies provided to affiliate. This is a condition of employment.
Excellent verbal and documentation skills
Ability to work effectively and cooperatively with clinicians and other professional and volunteer members of staff
Ability to demonstrate sensitivity and empathy to patients and family members
Knowledge of quality assurance and risk management standards
Bilingual skills a plus, particularly Spanish or Serbo-Croatian or Vietnamese
