Our facility offers behavioral and psychological treatment for adults and adolescents through our acute programs, adolescent residential programs, as well as programs for active duty military. Poplar Springs Hospital is located in Petersburg, Virginia, 20 minutes south of Richmond and 50 minutes north of North Carolina. We have a new opening for a RTC - LPN Care Coordinator. The LPN Care Coordinator will be responsible for directing and supervising nursing service activities to ensure quality patient care on the residential treatment units and for the communication and documentation of such functions and activities. Ensures safety of all nursing staff and patients/residents, provides nursing staffing levels in collaboration with the Staffing Coordinator and Director of Nursing Systems. Serves as role model and a resource for all staff.Transport patients/residents to/from appointments as scheduled by facility staff. In addition to providing transportation, this position will also be utilized to perform clerical duties such as but not limited to, filing unit/department supply requisitions, delivery of supplies to various locations and delivery of packages received through UPS, Fed Ed etc. During times when there are no transports scheduled and other obligations are fulfilled, this position may be asked to perform other clerical duties such as filing, copying, and duties as such or be assigned as MHW on a unit. Performs other duties as assigned.

Qualifications


Licensure/Certification/Registration:
      • Licensed to practice as Licensed Practical Nurse in the Commonwealth of Virginia
      • Valid Virginia state driver's license.
      • CPR Preferred (offered on-site)
      • Handle With Care Preferred (offered on-site)
Education/Experience:
      • Minimum of 3 years’ experience in nursing management position preferred.
      • Working knowledge of Joint Commission, OSHA State and Federal regulations.
      • High school diploma or equivalent required.
      • Previous experience driving larger sized vans or trucks preferred.
      • Previous experience transporting patients, residents or general public preferred.
      • Basic knowledge of state laws of the road.
General Requirements:
      • Must be at least 21 years of age per Commonwealth of Virginia Interdepartmental Regulation of Children’s Residential Facilities. (22VAC42-11-380, pg. 43)
      • Must have patience, tact, and a cheerful disposition and enthusiasm.
      • Good communication skills; must be able to read, write, speak, and understand the English language.
      • Basic clerical skills, basic experience with office equipment (telephone, copy machine, fax, etc.)
      • Experience with Microsoft Office Software (Word, Excel, PowerPoint, Outlook, etc.)
      • Ability to understand and carry out oral and written instructions.
      • Ability to problem solve and accept feedback when circumstances warrant such actions.
      • Willingness to interact with patients with varying levels of cognitive functioning.
      • Travel required.
