Position: Rural Health Clinic Primary Care Provider
Reports To: CMO & Practice Manager
Schedule: Full time days, plus rotational on-call
Position Location/Department: Sheridan Care
Job Summary:
Join our family medicine care team, providing full time primary care in a small community hospital setting. Enjoy the convenience of hospital services, lab, diagnostics and specialty services directly across the street. Form relationships with patients as you integrate into a team of established physician and PA with a range of capabilities including procedures, chronic disease management, medication assisted withdrawal treatment, sick visits, well child and adults. As APP you will provide patients with high-quality treatment. The ideal candidate is an intuitive healthcare professional whose diagnostic abilities are only matched by their concern for their patients. The provider in this role will be responsible for meeting, assessing and diagnosing patients in our clinic and providing them with healthcare services that meet state and national standards for caregiving. They will be responsible for prescribing patients with appropriate medication, health regimens and lifestyle adjustments that will contribute to their recovery from diagnosed health issues, prevent future health issues and/or promote a healthier life. The right candidate will have a passion for providing diagnostic services to patients and will enjoy working closely with other healthcare professionals in our organization.
Essential Duties and Responsibilities:
- Adhere to departmental policies, procedures and objectives, ongoing quality improvement objectives and safety, environmental, and infection control standards
- Administer medications and injections; sutures minor lacerations
- Attend required meetings; participate in committees as required
- Maintain continuous education requirements for licensing and current standards of care
- Patient visit documentation includes systematic review of past medical, surgical, family, and social history, problem list, medication/allergies, and discussions of new medication risks and benefits
- Effectively utilize consulting physicians and other diagnostic services that are not provided by the Clinic
- Establish and maintain effective working relationships with area health care entities such as hospitals, physician practices, health departments, home health care agencies, and other ancillary support groups
- Interpret and integrate data to determine appropriate diagnostic and therapeutic procedures as needed
- Interview and provide recommendations to patients regarding health and illness prevention. Recommend community resources to meet patient and family needs
- Maintain EMR/patient records by recording complete timely, and legible medical records
- Maintain patient confidentiality and comply with all federal and state health information privacy laws
- Manage medical emergencies occurring in the office setting
- May routinely perform such therapeutic procedures as (but is not limited to): debridement, suture and subsequent care of wounds; removal of impacted cerumen, subcutaneous local anesthesia, nasal packing for epistaxis; splint sprains and fractures; incise and drain localized abscesses and utilize cryotherapy to warts; and other procedures the APP is trained to perform and not limited by the medical director
- Collaborate with physician supervisor regarding management of patient needs
- Provide comprehensive primary care within the regulatory guidelines set by the State of Michigan and Rural Health Clinic Code of Federal Regulations
- Other duties as assigned within scope of licensing
Education, Experience and Other Requirements:
- Experience providing primary care (1+ year preferred)
- Excellent communication and interpersonal skills
- APP role has the following certification:
- PA, NP, or DNP degree with appropriate accreditation
- Licensed in the State of Michigan
- Current Drug Enforcement Agency (DEA) and Department of Public Safety (DPS registration, for prescription writing
