Southside Medical Center, Inc. (SMC) was founded in 1967 as the Atlanta Southside Comprehensive Health Center. We are a federally qualified health center (FQHC) that provides primary health care and related services to the medically underserved and uninsured.
SMC has 13 locations across the state of Georgia. Our main Center is located near the Georgia State Football Stadium in Fulton County. We also have two other locations in Fulton County: one at Dobbs Elementary School and the other in East Point. We have two locations in Dekalb County on Gresham Road and in Decatur, and three locations in Clayton County in Lovejoy, Riverdale and Forest Park. One location in Gwinnett County in Norcross, and a location in both Spalding and Butts Counties, Duluth and our newest location, Cleveland Family Medicine in East Point.

JOB DESCRIPTION
Under supervision of the Ryan White Program Manager performs evaluation and analysis of data; prepares routine reports documenting results of findings, create reports using MS Access, Excel, Word, or PowerPoint; schedule and reschedule Ryan White and PrEP patients.
DUTIES AND RESPONSIBILITIES:
1. Manage the CARE Ware and e2Fulton data collection software and prepare reports to assess the impact of Ryan White.
2. Serve as the Quality Management data designee for the Ryan White program.
3. Create and maintain spreadsheets on patient clinical and non-clinical information.
4. Design and maintain summary reports for staff use.
5. Review program reporting requirements and current data collection systems to ensure efficiency.
6. Input all data on services, labs, exams, transportation etc. into CARE Ware and e2Fulton for all Ryan White patient appointments.
7. Maintain and update patient records.
8. Make phone calls to schedule and reschedule Ryan White and PrEP patients.
KNOWLEDGE, SKILLS AND ABILITIES:
- Must possess professional knowledge of principles of HIV Care & Treatment, professional and cultural competence/sensitivity, compassionate attitude and be familiar with service delivery for HIV positive clients.
- Possess good knowledge of community resources and demonstrate a high level of customer service.
- Must have the ability to think critically and analytically, process information and prioritize, maintain confidentiality, and possess independent work habits, is self-reliant and self-directed, knowledge of EMR databases, MS Office Suite and use of evaluation methods to report on program outcomes.
- Participate in the planning and implementation of Ryan White goals and objectives. Candidate should have strong interpersonal and communication skills (both verbal and written) and the ability to function independently under dynamic circumstances.
- Strong attention to detail and the ability to manage multiple projects, establish workload priorities, and meet deadlines are critical.
- Proficient computer skills (MS Office); expertise in establishing and maintaining effective working relationships with staff and partner agencies; skill in establishing rapport and having empathy with clients; contribute to a team-based approach focused on patient care.
MINIMUM QUALIFICATIONS:
Bachelor's Community or Public Health preferred. Prefer minimum of two years related experience working with high-risk targeted populations required (HIV Counseling, Expanded/Targeted Testing, Referral, linkage and retention of clients in HIV and/or care/treatment). Familiarity with public health program implementation and evaluation; knowledge of the use of motivational interviewing techniques to encourage individuals to start and remain in care.
TYPICAL WORKING CONDITIONS:
Administrative and medical office environment with potential exposure to communicable diseases, bodily fluids, toxic and other substances common to clinic environment. May involve work under stress in emergency situations or during irregular hours.
