Your Specific Responsibilities:
This is a professional nursing supervisory position with responsibility for appraisals, planning and directing work with authority for hiring and taking disciplinary action or effectively recommending such actions. This position is the primary level of authority in the chain of command for nursing in the Coastal Branch and Paxton clinics and reports to the Sr. Community Health Nursing Director. This position has the responsibility to provide nursing services through Primary Care, Family Planning, Immunization and STD clinics as needed. This position is responsible for direct supervision of nursing staff at the Coastal Branch and Paxton sites. The person in this position will be performing tasks involving sensitive information and requiring a high level of trust. Background screening, drug testing and fingerprinting are required. This position acts as key custodian for secured areas including the laboratory and medication rooms and will adhere to the Department’s information security policies, protocols, and procedures. This position will function as back-up support and provide onsite supervision or direct nursing services at other clinic sites if needed. This position will provide direct care and nurse clinic coverage as needed.
This position will follow Florida Department of Health, Florida Department of Health Walton and Walton Community Health Center policy and procedures. This position is a key custodian for assigned areas.

This position may assist in the provision of care coordination:
- Link patients with community resources to facilitate referrals and respond to social service needs.
- Track and support patients when they obtain services outside the practice.
- Obtaining test and referral results and communicating with community organizations, health plans, facilities, and specialists.
- Follow-up with patients within a few days of an emergency room visit or hospital discharge.
- Communicate test results and care plans to patients/families.
Supervisory duties include oversight and guidance of staff to assure productivity and efficiency, compliance with FDOH Walton and Walton Community Health Center policies, procedures and practices, and use of computer systems and programs. Direct supervision includes regular, required evaluations, annual review of position descriptions for accuracy and skills assessments to evaluate performance. Encourage staff to participate in personal and professional growth activities such as trainings, education, staff meetings and committee participation. Acts as a positive role model to co-workers and other DOH staff. Pulls reports from HMS or other systems to monitor activities such as numbers of services provided, timeliness of staff documentation, and quality of documentation. Assures that new employees receive adequate training in role(s) and acts as a coach/mentor to those supervised. Takes a forward-thinking approach to programmatic needs and has a “can-do” attitude for problem solving. Approves time sheets for staff and monitors staff EAR records to assure timeliness and accuracy. Is part of the Clinic Management Team and works to assures that all clinic locations operate within same guidelines and expectations.
Management duties include assuring that staff have opportunity for adequate training related to individual positions. Assures that records reviews and related reports are performed in a timely manner and reported as appropriate. Provides reports to quality committee in approved format and attends quality meetings as needed to report on program activities. Reviews reports and records related to financial issues to assure fiscal sustainability. Monitors budget and expenditures to assure a balance between income and expenses. Strives to provide a work environment that promotes positive teamwork. Assures that all programs, including Primary Care, Family Planning, Immunizations, STD, and all other clinic programs, including the UF Residency Program operate within established guidelines. Assures that all equipment is safe, in good condition, calibrated regularly if needed and operates as it should. Assures that staff maintain current Florida nursing licenses and BLS certifications. Assures that new employee orientation is adequate and updated as needed. Holds regular staff meetings that document attendance and topics addressed. Promotes high productivity and efficiency in clinic practices. Backs-up all assigned staff and provides direct client care as needed to ensure clinic services are interrupted as little as possible.
Is responsible for the oversight of laboratory activities to assure that CLIA license is current, renewed timely and is displayed appropriately. Assures lab equipment is safe, in good condition, calibrated regularly if needed and operates as it should. Assures all Laboratory Quality Assurance activities are completed based on manufacturer’s recommendations. Assures all Laboratory, Policies, procedures, protocols and processes are reviewed and updated at a minimum of every 2 years. Is responsibility for the oversight of all medication room activities to include ordering pharmaceuticals, receiving orders, stocking supplies, documentation retention and maintaining Inventory and Inventory Systems. Is responsible to assure that clinic supplies are available as needed to conduct business as usual.
Will be assigned as a VFC Back-up Coordinator for the Coastal Branch location. Assures that every opportunity for immunization is utilized. Immunizations will be provided to both children and adults. Assures that immunization information is disseminated to staff as needed. Assures that temperature monitoring materials and reports are reviewed, submitted, and maintained. Has extensive knowledge of all DOH sponsored Immunization programs. Monitors for program compliance. This person functions as a immunization authority and manages reports, orders vaccines and assures accurate inventory of vaccines.
Works with Medical Director and Director of Nursing to assure that programs are monitored as planned and that results, reports and summaries are documented. Reports are submitted to appropriate committees and improvement actions are implemented as indicated. Assures that patient education including discharge teaching is performed in accordance with provider and agency expectations. Assures that documentation is accurate, appropriate and concise including client understanding and who provided education. Facilitates a robust QI/QA program that embraces standardization as much as possible. Utilizes the “Plan, Do, Check, Act” cycle when improvements are required.
Provide community education as requested. Participate in outreach activities that support FDOH – Walton programs
and projects. Maintain current Florida registered nursing license, maintain current CPR certification. Attend appropriate staff meetings. Participates in committees and workgroups as requested (time permitting) to assure program representation throughout the health department. Attend conferences and trainings to enhance job skills. Complete time sheets and employee activity records according to agency standards. Duty during times of disaster are required as indicated to meet the needs of the citizens of Walton county. May act as a resource for community-based programs under supervisor’s authority as requested. Perform other related duties as assigned. May be required to use personal vehicle for official state business. May be assigned purchasing card responsibilities. Will adhere to purchasing card policies and procedures.
May be assigned the afterhours on-call for client phone calls, abnormal lab result notification and urgent follow ups.

Other knowledge, skills and abilities:
Working knowledge of all DOH- Walton and Walton Community Health Center’s clinical policies, procedures and/or
protocols. Ability to plan, organize and coordinate work assignments. Strong time management skills. Ability to remain flexible and adaptable in the work environment. Experience in crisis intervention and/or teaching coaching. Ability to evaluate situations, develop an action plan and adjust activity and behavior to meet goals and objectives. Ability to learn and utilize a variety of computer programs such as Outlook, Word, Excel, and HMS. Must be able to lift and carry 25 lbs
Qualifications:
Minimum - Two years of experience as a Community Health Nurse
Preferred - A bachelor's degree from an accredited college or university with a major in nursing or a related field and two years of professional nursing experience, including one year of pediatrics or public health nursing, and licensure as a Registered Professional Nurse in accordance with Florida Statute 464
